Medication Management Services

Residential Medication Management Review (RMMR)

For residents of approved aged care facilities, our RMMR service delivers a comprehensive, GP‑referred medication review. Our credentialed pharmacist systematically evaluates all medicines, identifies and resolves medication‑related problems, and delivers a detailed report with recommendations to optimise therapy and reduce risk.

Quality Use of Medicine (QUM) Audits & Support

We help aged care homes embed safe, effective medicine practices through facility‑wide QUM audits, advice and education. This includes reviewing prescribing and administration procedures, supporting governance and compliance, and guiding safe use of high‑risk medicines.

Aged Care Onsite Pharmacist (ACOP)

Under the ACOP Measure, our pharmacists can be embedded on‑site at residential aged care homes, acting as part of the care team, available for day‑to‑day medicine oversight, medication reconciliation, clinical support, and continuous quality use of medicines activities. This model promotes real‑time oversight, improved safety and ongoing pharmacist input.

Home Medication Review (HMR)

For older adults living at home, our HMR service offers a structured, GP‑referred medication review. Our pharmacist works collaboratively with the patient (and carer if relevant) to assess medicine use, identify issues, improve understanding, and provide a comprehensive report to the GP and care team, helping to reduce adverse events and ensure effective, safe therapy.

Who can refer a resident or patient for a medication review?

A resident’s GP or authorised medical practitioner can refer aged-care facility residents for RMMR, or community patients for HMR. Facilities may also request QUM audits; aged care homes interested in embedded pharmacist services may engage via ACOP.

What issues can a review identify?

Medication reviews help uncover a range of issues including unnecessary or duplicate medicines, inappropriate dosing, harmful interactions (especially in polypharmacy), potential adverse drug events, medicines no longer indicated, and opportunities to simplify medicine regimens.

How often should reviews be done?

For residential care, an initial RMMR at admission is recommended, with follow-up reviews when there’s a change in clinical condition or medicine regimen. For home-based patients, review frequency depends on clinical need, medicines complexity, and GP recommendations.

What is the difference between ACOP and RMMR/QUM?

RMMR/QUM are episodic, include visiting pharmacists conduct scheduled reviews or audits. ACOP, on the other hand, embeds a credentialed pharmacist on-site at the facility, offering continuous medication oversight, real-time clinical support, and integration with care-team workflows.
