Home Medicines Reviews (HMR)

Medication Reviews for Community-Based Patients

We assist you in arranging HMRs for older patients who are managing multiple medications, recovering from hospitalisation, or struggling with adherence or understanding.

You can submit referrals securely via email, fax, or online form. No additional paperwork or back-and-forth is required from your end.

We handle the home visit, conduct a full medication assessment, and identify safety concerns, confusion, or gaps in knowledge.

We assess all prescribed medicines, supplements, OTCs, and non-pharmacological treatments in the context of your patient’s history and living situation.

You receive a structured, clearly written report with findings and recommendations, ready for you to review and action as appropriate.

We can follow up with carers, community nurses, or pharmacists where needed — always keeping you informed.

We ensure the review is clinically justified, PBS-compliant, and conducted by an AACP-accredited pharmacist.

Residential Medication Management Reviews (RMMR)

Pharmacist Support for Your Aged Care Patients

We support you in delivering RMMRs for patients living in residential aged care, ensuring clinical depth and full PBS compliance.

We coordinate directly with the facility and pharmacist , including consent, scheduling, and documentation, so your team doesn’t carry the admin load.

You remain central to all decisions, our role is to support your prescribing with evidence-based, resident-specific recommendations.

We assess every medication, including PRNs and supplements, and identify interactions, duplication, or deprescribing opportunities.

We send the completed report directly to you, clearly formatted, easy to review, and aligned with RACGP and PBS guidance.

You’ll receive a notification if a follow-up is needed after hospital discharge or changes to your patient’s condition or therapy.

If required, we can communicate with the facility or family to help implement your agreed changes.

Prescriber Training

Medication Review Support for Prescribers

We offer short, practical education sessions focused on medication reviews, referral criteria, deprescribing, and medication safety.

You’ll learn how to identify ideal patients for HMR/RMMR, avoid PBS pitfalls, and better manage polypharmacy and high-risk medicines.

Sessions are 30–60 minutes and can be run in your practice, online, or via live webinar, whatever suits your schedule.

You’ll have the chance to ask questions and workshop real examples, not just hear theory.
