Residential Medication Management Reviews (RMMR)

Funded Reviews for Safer Prescribing

Funded under the RMMR program, we provide comprehensive medication reviews for aged care residents in collaboration with your GPs.

Our focus is on identifying unsafe combinations, outdated or unnecessary medications, and missed opportunities to simplify therapy or deprescribe.

We assess all prescribed medications, over-the-counter products, and supplements including PRNs in relation to the resident’s health status and care goals.

We provide structured reports written in plain English for GPs and facility staff, including evidence-based recommendations aligned with PBS and national guidelines.

Our reviews help prevent adverse events such as falls, sedation, and confusion while supporting clearer care planning and communication.

We offer follow-up reviews after hospital admissions, major health changes, or increases in medication complexity.

Every review is delivered by an AACP-accredited pharmacist and supports compliance with Aged Care Quality Standards 3 and 8.
